I request that my service, located at the address listed below, be connected or reconnected and agree to be in the Dixie Power office by 10:00 a.m. the next regular business day to complete the application transaction. I also agree to pay with CASH or CREDIT CARD (Visa, MC, Disc, Amex) the after-hours fee of $75, plus any other fees associated with connect or reconnect.
